about AFCS

1. I need to see someone immediately, what should I do?
AFCS does not provide crisis intervention; however, we will do our best to accommodate your needs. If you need immediate assistance and we are not able to accommodate your needs, you may call the Crisis Center at (920) 437-8888.

2. What is counseling?
Counseling is a process where the client and the counselor examine current problem areas and strengths to develop a plan to improve the areas where improvement is needed.

3. Are there different styles of counseling?
Yes. Some counselors draw out answers from the client using discussions while some counselors give more information. Certified counselors all have training in techniques which are used to improve the mental health of the client. It is important to find a counseling style that works well for you.

4. What happens when I get there?
Counselors will gather information on your current feelings and behavior and help you clarify your treatment goals. Gathering individual and family history is part of the first session.

5. Can I go in with my son or daughter?
Counseling for children and adolescents often includes a blend of family and individual therapy. Parents are part of the first session where gathering background information is needed. Parents are then involved to the degree the counselor sees as helpful in achieving the agreed upon goals.

6. What services do you provide?
AFCS has Counseling Services and Group Therapy, Foster Care as well as Clergy and Congregational Care.

7. Do you provide faith based counseling?
Each therapist is educated in the process of integrating spirituality into therapy. Therapists assess each client's spiritual values and resources. The client's own faith values are used when designing the client's therapy goals.

8. Who are your providers? What are their credentials?

9. Do I need a referral?
We do not require a formal referral from a medical provider or another specialist. We may ask if you were referred to AFCS by anyone. We will only use summary data for grant writing and development purposes; you will not be identified in this information if you do provide your referral source.

10. What are your hours?
Normal business hours are Monday - Friday 8 a.m. to 5 p.m.; additionally there are evening hours Monday and Wednesday until 6:30 p.m. These hours are subject to change. Please feel free to call the Crisis Center at (920) 436-8888 if you need immediate assistance and it is outside our normal business hours.

11. What are your rates?
Fees for therapy are $170-$195 for an initial 50 minute diagnostic evaluation, and $135-$160 for either a 45 or 60 minute session. Psychological testing is $185, and group therapy is $67.50.

12. Will my insurance pay for services?
Many of our services are insurance billable and, as a service, we will bill your insurance. You are responsible, however, for ensuring our services and therapists are covered by your insurance. Please contact your insurance company for a complete description of your out-patient mental health services benefits and eligibility. Please refer to your insurance card for contact information details.

13. What if I don't have insurance?
Payment alternatives may be available for patients who do not have insurance. If you are uninsured and would like to discuss alternatives, please have your income information available and contact our Intake Coordinator at (920) 437-8256.

14. I have a high deductible insurance plan and can not afford to pay the regular rate at each session. Can I still be seen?
Yes. We will work with you to establish an affordable payment plan. If you would like to discuss your account, please call (920) 437-8256.

15. Do you accept Wisconsin Forward Insurance (Medicaid)?
Yes.

16. Are you a state licensed facility?
Yes, we are a State Certified Outpatient Mental health Clinic which is the Wisconsin licensing terminology.

17. What happens if I can't come to an appointment?
Please notify our office, (920)437-8256, at least 24 hours in advance if you are unable to keep your appointment. This courtesy makes it possible to give your appointment time to another client who would like to see a therapist. If you do not notify our office at least 24 hours in advance, we reserve the right to charge you for the missed appointment.

18. How do I obtain my medical records from services provided by an AFCS provider?
You need to fill out and sign a Release of Information Form and return it to our office.
